Welcome to The Avenue Nursing and Rehabilitation Centre located on 526 Altamont Avenue Schenectady, New York. Our dedicated employees chose the name The Avenue due to our location on a major avenue in Schenectady. When it comes to making health care decisions, there are many different “avenues” one can take. For the following reasons, we hope you explore the services offered at “Our Avenue.”


Services Offered

- Our 162 bed licensed skilled nursing facility offers both short and long- term care.

- We offer short-term rehabilitative services to people who cannot immediately return to their homes after a hospital stay. Our rehabilitation unit is conveniently located next to our rehabilitation department and separate from our traditional long-term care units. Our dedicated rehabilitation staff, including physical, occupational and speech therapists, have successfully discharged many of our resident’s back into the community to resume independent living.

- We also offer traditional long-term, skilled nursing care for people who can no longer care for themselves at home, and choose to make us their new home.

- We offer full time, on-site physician services to address more complex medical needs.

- We provide individualized meal plans and our residents are able to choose their favorites from our seasonal menus.

- We offer a wide variety of group and individualized activities. Residents will enjoy outside activities such as boat excursions, restaurant and shopping trips, and a day at the Saratoga Race Track. A full calendar of activities is provided monthly.

- We provide several sitting areas for residents and loved ones to visit in, including an outdoor patio and a spacious beauty parlor.

- We accept Medicaid, Medicare, secondary insurances, long-term care insurances, various HMOs and private paying residents.
