When you entrust the care of your elderly loved ones to others, you want to know that they will be cared for by professionals with the expertise to meet their special needs with dignity and respect. At Medina Memorial’s Residential Health Care Facility, resident’s rights have been fully defined to ensure that:
* residents are treated with dignity, self-determination, and respect
* residents are given full recognition of their individuality
* residents are given consideration and privacy in treatment
* residents have all personal needs are met
* residents are provided with communication to persons both inside and outside the facility.
Staff actively promote these rights, and encourage and assist each resident in the fullest possible exercise of them.
Each member of our residential health care team is involved in the resident’s personal care as an extension of each resident’s family. Individual care plans are developed according to the specific needs of each resident.
Our activities program is designed to increase the quality of life for each of our residents by allowing them to be as active and involved as they wish. Activities are offered daily, and residents can choose which to attend.
To help ensure the highest standards of care, quality indicators are monitored on an ongoing basis, with data submitted to the New York State Department of Health for comparison with peer group performance. We also voluntarily participate in surveys by the Joint Commission.
Joint Commission accreditation is recognized nationwide as a symbol of quality that reflects an organization's commitment to meeting certain performance standards. To earn and maintain accreditation, an organization must undergo an on-site survey by a Joint Commission survey team at least every three years. When improvement opportunities are identified, teams are formed to look at care processes and identify changes that promote high quality care.
In addition to residential health care, there are people who may need skilled nursing care between treatment provided in the hospital setting and their return to home. Medina Memorial’s Residential Health Care Facility can provide the rehabilitative care they need to make that transition.
