Greenfield Health & Rehabilitation Center is part of The GreenFields Continuing Care Community. This 162-bed facility, which opened in 1998, complements the residential apartment homes and assisted living services offered by GreenField Manor and GreenField Court on our beautiful 52-acre campus in Lancaster, New York.
GreenField Health & Rehabilitation Center offers:
• Skilled nursing care
Our skilled nursing residents receive the unique day-to-day services they require so that they can live the highest possible quality of life available to them, with dignity. Admission to GreenField Health & Rehabilitation Center is the beginning of a new life for many of them. We take great care to make an attractive, homelike environment, and we encourage our residents to socialize. We welcome their loved ones to spend quality time with them.
In addition to medical care, emphasis is placed on quality of life and spiritual and emotional support. The depth of compassion and caring associated with the delivery of quality care is the main attribute, which differentiates one skilled nursing facility from another. Our residents become part of the Niagara Lutheran family.
• Subacute care
Whether you are recovering from surgery, wounds, injuries or other medical conditions, you will receive compassionate and professional care at GreenField Health & Rehabilitation Center. Team members from various departments will work closely with each other, with you, and with your family to ensure the most complete recovery possible.
We are committed to helping each individual reach his or her potential and will help with arranging for necessary services and equipment upon discharge.
• Respite care
Caring for a loved one suffering from a debilitating illness is an exhausting and sometimes over whelming burden. Respite care is available for an individual who is normally cared for at home. This provides the caregiver a well-deserved rest and/or vacation, secure in the knowledge that their loved one is receiving a full range of nursing services during your absence.
Respite residents are encouraged to participate in and utilize all health center activities and services during their stay with us.Their rooms are adjacent to sitting areas that create a homelike atmosphere.
• Rehabilitation services
Our highly trained and compassionate staff is available to assess your particular situation and determine the best treatment for you. We will communicate with you, your physician, and your insurance carrier.
• Outpatient therapy and wellness programs:
â—¦ Including specialized clinics for amputees, the wheel-chair bound, sports enthusiasts, and others as needed.
At the GreenField Health & Rehabilitation Center, every resident benefits from an individualized plan of care developed and carried out by an interdisciplinary team that meets regularly to discuss progress and update the plan of care as necessary. The team is comprised of the resident and his or her family members, as well as our medical director; physical, occupational and speech therapists; and nursing, activities, nutrition and social services staff. If it is determined that a resident is able to go home or to a different level of care, our social services staff will help with discharge planning and arranging for community services and special equipment that might be necessary.
The GreenField Rehabilitation Department has earned a reputation as the premier provider of rehabilitation services in Western New York. Rehabilitation and subacute care patients are cared for on their own special wing so they receive the privacy and specialized nursing care they need. Our staff physician meets regularly with patients to ensure that they are progressing to the best of their ability. Many patients find that the familiarity of staff and procedures when they continue with outpatient therapy at The GreenFields helps them to achieve the best possible results.
