Cold Spring Hills is 606 bed nursing facility located in Nassau County. Surrounded by an enclosed two-acre garden courtyard, on a twelve-acre campus are five resident care buildings, the largest of its kind on Long Island. We provide health care services through a continuum of care from, our signature short-term stay Passport Rehabilitation Program, to Adult Day Health Care Programs, Long Term Home Health Care Program, and in-patient Skilled Nursing Care. As a result of our size and commitment, we can provide a homelike environment with enhanced accessibility and socialization, utilizing mostly private rooms.
Most recently we have become an affiliate of the North Shore/LIJ Health System, the third largest of its kind in the United States. Our clinical affiliations with the North Shore/LIJ Health System, and Winthrop University Hospital allow us the distinction of being a state of the art teaching organization.

Providing the highest quality of life through an ongoing continuum of care, Cold Spring Hills offers both Long Term Home Health Care and Adult Day Health Care Programs along with our residential programs
Long-Term Home Health Care Program
Since most people wish to remain at home for as long as possible, we have a program to make that wish come true. Our Long Term Home Health Care Program (LTHHCP), also known as “Nursing Home Without Walls†and the Lombardi Program, provides patients and their families with a popular alternative to long term care. A comprehensive package of coordinated services is designed to meet the specific needs of patients and to provide visits and ongoing care in their homes. The hallmark of this program is case management by a Registered Nurse.

Adult Day Health Care Program
At Cold Spring Hills our Adult Day Health Care Program promotes maximum functioning and health capabilities of clients while contributing to their self-esteem. The program offers:

â—‹ Open seven (7) days a week from 9:00 a.m. to 3:00 p.m.
â—‹ Door-to-door service in handicapped accessible vans
â—‹ Individualized health/medical regimes based on need, ability and interest
â—‹ Providing comprehensive services to individuals with medical and emotional
issues including Alzheimer’s Disease.
â—‹ High staff-to-client ratio for greater individualized attention
â—‹ Supervised community outings and social events
â—‹ Arts and crafts, games, music, entertainment and socialization
â—‹ Activities of daily living to encourage self-sufficiency
â—‹ Daily hot lunches and snacks
â—‹ Physical, occupational and speech therapies as prescribed
â—‹ Administration and/or monitoring of medications
â—‹ Tube feeding, colostomy care, wound care, suctioning, tracheostomy care
