Clove Lakes provides a full range of services to individuals who are in need of skilled nursing, therapeutic and / or rehabilitation care. Our success in providing compassionate quality care is evidenced by the excellent reputation we enjoy in our areas of specialization, including:

- Outpatient Rehabilitation Center for individuals who are injured or those getting back to an active lifestyle following orthopedic surgery and illness.

As a result of delivering a consistently high level of resident and family satisfaction, Clove Lakes Health Care and Rehabilitation Center has been chosen as a teaching facility for nursing students, nursing assisstants, physicians assistants, medical residents of physiatry and internal medicine, as well as physical, occupational and speech therapy programs. We also participate in a fellowship program for medical residents.

SHORT TERM REHABILITATION

Short term rehabilitation provides care for patients with a wide range of rehabilitation needs. The majority of patients have orthopedic, neurologic, neuromuscular or arthritic disorders that may include stroke, fractures, pain management, amputation, brain injury, Multiple Sclerosis, or Parkinson's Disease. More than 25 percent of all admissions have cardiac-related diagnoses and need continued skilled care following surgery or illness.

Candidates for short term rehabilitation have medical complications but also have the potential to improve with a comprehensive rehab program. Rehab addresses physical and/or medical problems, difficulties with activities of daily living and specialized problems such as dietary management.

Three dedicated nursing units totaling 126 beds comprise the short-term rehabilitation program. Short term rehab patients are placed on these special units with their own clinical staff and support staff in order to achieve greater clinical focus and superior rehabilitation outcomes. Rehabilitation is provided in a modern, fully-equipped 4,000 sq. ft. Rehabilitation Gymnasium.

Upon admission, each patient is carefully reviewed by an interdisciplinary treatment team which includes a board-certified physiatrist, a physician specializing in physical medicine and rehabilitation. As part of its individualized approach, each patient is re-evaluated on a regular basis. This leads to a therapy program that continuously grows and changes with the patient, always encouraging progress to achieve the highest level of independence.

In addition to a physiatrist, team members may include consultants in neurosurgery, orthopedics, neurology, internal medicine and other specializations, along with a physical therapist, an occupational therapist, a speech pathologist, a social worker, a rehab nurse, dietitian and recreation specialist.

Keeping the family informed is a high priority of the interdisciplinary team and regular family conferences are held to expedite recovery and aid the transition to home.


LONG TERM SKILLED NURSING CARE

The 1967 opening of Clove Lakes made the dream of founder Nicholas Demisay a reality. Since that time, the Clove Lakes name has been synonymous with excellence. More than 40 hospitals from the greater New York area admit their patients to Clove Lakes and the thousands of residents and family members who have experienced Clove Lakes know they have received the most comprehensive, technically-advanced health care services available.

Upon admission, each resident is evaluated by our interdisciplinary team. The team develops a comprehensive care plan which includes all medical, nursing, rehabilitation, nutritional, psychological and therapeutic needs. The resident, along with family members, actively participates in the planning process. Each care plan is individualized, taking into account a resident's likes and dislikes, hobbies, interests and functional capabilities.

Our dedicated social service staff serves as both an advocate and advisor to the resident and family members. We are well aware of the difficulty residents and families have when long term care placement becomes a necessity. Because of this difficulty, social workers make the transition period their top priority. A social worker is assigned to each resident on the day of admission. This individual is also available to assist family members.

Each resident is evaluated upon admission for rehabilitation services which include physical, occupational and/or speech therapy. Based on the residents' needs, a plan of care is developed and carried out by a team of therapists. Our goal is to assist each resident to maintain his/her optimum level of functioning.


POST-HOSPITAL CARE SERVICES

Post-hospital care at Clove Lakes offers patients many benefits including a comprehensive medical rehabilitation program. Synonymous with excellence in rehabilitation services, Clove Lakes utilizes state of the art equipment and the most current therapy protocols to ensure successful outcomes for our patients.

Another benefit of post-hospital care at Clove Lakes is the outstanding discharge rate of the medical rehabilitation program.

In the first half of 2002, 89% of the patients who completed the comprehensive medical rehabilitation program were discharged home.

The Comprehensive Medical Rehabilitation Program admitted patients requiring rehabilitation for various conditions including knee replacements, total hip replacements, hip fractures, strokes, pulmonary and cardiac conditions, and muscle disorders.

We are proud of our continued success in rehabilitation and our discharge rate back to home. And, more than 40 hospitals from New York and New Jersey have referred six thousand patients since the program started in 1992.

Post-hospital services are ideal for persons who continue to require skilled nursing and rehab services with a focus on restorative care. On admission, the interdisciplinary team develops a plan of care to meet the patient's medical needs and ensure a quick return to the community.

- Medical Back Problems/Orthopedic Traction

Family participation is extremely important in the healing process. We encourage family members to visit regularly and provide encouragement for the resident. Each week, the rehabilitation team will meet to discuss the patient's progress and answer any questions.


RESPITE CARE

Our scheduled short-term respite care will be offered as an addition to our current long-term skilled nursing care and short-term rehabilitation programs.

Respite is often referred to as a gift of time because it provides relief to the caregivers and quality care to the patients. At Clove Lakes, we recognize that caregivers need to make time for themselves in order to relieve stress and prevent burnout. Caregivers are then able to return to their daily routine with a renewed vigor to continue to care for their loved ones at home.

Clove Lakes responded to the inquiries in the community for this service by designing two comfortable respite suites that will provide 24-hour care to applicants seeking a limited stay in our facility. Respite care will be offered on a weekly basis but cannot exceed 42 days in a calendar year.

Applicants must be 16 years of age or older and are required to provide a full medical history together with a physician's order for the scheduled short-term stay.

A full comprehensive medical assessment will be performed by the Clove Lakes staff on all patients admitted to the program.

ON SITE HEMODIALYSIS

We are proud to announce the opening of the only on-site Hemodialysis unit in a skilled nursing facility on Staten Island.

This 4 station state of the art hemodialysis unit administered by Island Rehabilitative Services, Inc. under the leadership of Dr. Morton Kleiner, Medical Director, will provide comprehensive dialysis services for our in-house residents and for outpatients from the community.

Comprehensive Professional Services include:

- Experienced dialysis RN's / LPN's / Technicians
- Certified Social Workers
- Registered Dietitians
- Physician on call 24-hours a day / 7 days a week
- Capacity to provide dialysis treatment for 32 people
