Blossom View is an independently owned skilled nursing and rehabilitation facility in Sodus, NY offering both residential and outpatient services.

Our facility is situated entirely on one level with easy access to any part of three residential sections. Private and semi-private rooms accommodate 130 residents in air-conditioned rooms. All are equipped with basic furniture. Cable services are included; phone services may be ordered. Wireless internet is available through out the facility. A security system provides a safe and secure environment.


REHABILITATION

Blossom View offers rehabilitation services to patients in residence and to the community on an out-patient basis.

The goal of rehabilitation services is to assist persons to reach their highest potential of independent functioning. Individual goals vary depending on their unique situation. For many who are post surgery, the objective may be to exceed the level of functioning they had prior to their surgery.

Blossom View offers physical, occupational and speech therapy services. Physical therapy is designed to address a person's ability to walk, to be mobile, remain steady and transfer from sitting to standing to prone positions. Occupational therapy assists patients to attain or retrain skills for dressing, eating and leisure activities. Speech therapy is available for those who need to redevelop or enhance their ability to communicate and eat.

These rehabilitation services are physician ordered to address a variety of chronic or acute situations. A comprehensive rehabilitation plan for residents and short term patients requires the coordinated effort of the entire Blossom View professional team. Additional services may include range of motion or other prescribed services through the nursing department, aqua therapy, counseling, nutritional intervention, specialized equipment and case management services.

Our unique dining room ambulation program enlists designated residents to walk with whatever assistance is needed to their selected dining room seat.

Our professional supervised therapeutic dining program provides assistance and training for residents to regain independence in dining and ensure their dietary needs are met .This unique program is a key component of our restorative program.

Many people are electing to have orthopedic surgeries to improve their quality of life. (Hip or knee surgery for example). The medical community recognizes that some post surgery patients benefit from a short term rehabilitation stay. Hundreds of patients have come to Blossom View for just that.


ALTERNATIVE THERAPIES

More than Medicine

Over a lifetime, people learn for themselves how to manage their emotional, spiritual and physical health. Our habits and self prescriptions help us remain balanced, calm, pain free, and relaxed. They have little to do with traditional medicine. Blossom View recognizes the potential benefits of alternative modalities to address such issues as pain, medication reduction, agitation, insomnia, and over all psychological and physical well being. As such, over the last year, we have been working on developing programs that offer alternative treatment. They are now available upon request.

The New Modalities

Reiki is an ancient Japanese healing discipline and a form of touch therapy. It is very relaxing and is used to accelerate and strengthen any healing process after surgical procedures, assists in reducing stress, relieving pain, headaches and anxiety.

Aromatherapy is a holistic treatment based on the external use of essential aromatic plant oils to maintain and promote physical, physiological, and spiritual wellbeing. The essential oils may be used in massage, added to a warm bath, used to moisten a compress that is applied to the affected part of the body, added to a vaporizer for inhalation, or diffused throughout a room.

Therapeutic massage involves the manipulation of the soft tissue structures of the body to prevent and alleviate pain, discomfort, muscle spasm, and stress; and, to promote health and wellness. Various forms of massage therapy improves functioning of the circulatory, lymphatic, muscular, skeletal, and nervous systems and may improve the rate at which the body recovers from injury and illness.

Art Therapy is a mental health profession that uses the creative process of art making to improve and enhance the physical, mental and emotional well-being of individuals of all ages. It is based on the belief that the creative process involved in artistic self-expression helps people to resolve conflicts and problems, manage behavior, reduce stress, increase self-esteem and self-awareness, and achieve insight.

Music Therapy is an allied health service which uses music as an intervention to promote wellness, manage stress, alleviate pain, express feelings, enhance memory, improve communication and promote physical rehabilitation.


OUTPATIENT REHABILITATION SERVICES

In 1998 Blossom View opened its outpatient clinic. Services are provided by licensed and certified physical, occupational and speech therapists who work closely with patients, family members and referring professionals to establish and achieve rehabilitation goals.

The following services are provided by licensed and certified staff:

- Occupational Therapy
- Speech Therapy
- Physical Therapy

Referrals for service must be ordered by your physician. To make an appointment, call the direct out-patient line (483-2000). Our staff will assist you with your questions and set up your first appointment.

In 2007, there were 3886 incidents of outpatient services delivered. On average, there were 30 new patients referred to the facility every month.

Out- patient services are offered in a separate rehabilitation room for patients from the community. The building is totally handicap accessible. Parking is free. We have several therapists on staff enabling the clinic to accommodate appointments from early morning through early evening.

Rehabilitation Services may be covered through insurances, Medicare, Medicaid and private pay.


PHYSICIANS

Several local physicians have admission and attending privileges at Blossom View. These primary physicians make periodic visits to residents on site.

If there is a need for hospitalization, residents are transferred per physician order to the hospital of the resident's choice.

On site services in the following specialties are also available to residents:

- Audiology services
- Dental services
- Optometry services
- Psychiatric services

Our contracted pharmacist delivers medications daily.

In addition, all necessary equipment, including wheelchairs, can be obtained and delivered conveniently to our facility.


NURSING

The services of qualified dedicated nurses and certified nursing assistants are the backbone of any nursing and rehabilitation facility. They deliver the hands-on care to patients and residents. We are proud of our high standards and our full complement of nursing staff.

Unlike some other facilities, Blossom View never hires outside contracted nursing services. And all nursing assistants are New York State certified nursing assistants. The Director and Assistant Director of Nursing supervise a full complement of Blossom View nursing employees who know our patients/residents personally. Their offices and all administrative nursing offices are located in the residential areas. There is no "administrative wing" at Blossom View.

Each residential section (Baldwin, Cortland and Empire) has a nurse manager who coordinates the staff and services in his/her area. Her staff consists of registered and licensed practical nurses as well as a team of certified nurses aides 24/7. Maintaining the same staff in the same residential areas promotes the concept of continuity of care. Residents/patients are more comfortable if they are given consistent care by the same caregivers.

Blossom View is certified by the State of New York to teach the Certified Nursing Assistant course. This course is offered several times per year. We are, in addition, a training site for BOCES.


SOCIAL WORK

Professional social workers provide counseling, case management and support services to residents, patients and families. You will know your social worker upon admission. Please feel free to contact her with any questions or concerns.

The social workers focus their assessments and interventions on the social, emotional and spiritual aspects of the resident.

Social workers:
- Facilitate the resident and family councils
- Complete Admission procedures/paperwork with residents and families
- Plan and coordinate Family Day when families meet with the interdisciplinary team
- Coordinate psychiatric consultant services
- Develop and arrange discharge plans with the resident, family and Blossom View team
- Arrange period clothing day sales at Blossom View


FACILITY/ROOMS

Blossom View is situated entirely on one level with easy access to Baldwin, Cortland and Empire, our three residential sections. Private and semi-private air-conditioned rooms accommodate 129 residents.

Each room is furnished with the following:
- Bed
- Two Chairs
- Nightstand
- Mirrors
- Dresser
- Wardrobe
- Over bed stand
- Overhead lighting
- Television stand or desk

Cable services are included; phone services may be ordered. Wireless Internet is available in all rooms. A security system provides a safe and secure environment.
Unlike many other nursing facilities, Blossom View is staffed in all departments by our own employees. Laundry for the facility and each resident is done on site with our own staff. Our maintenance staff provides all the day to day upkeep and repairs. Food service and preparation is also done on site. In fact, Blossom View contracts out very few services, and we never use a temporary service in any department including nursing. It is our way of insuring quality services are available, secure and managed.


DINING

We pride ourselves with a modern kitchen, several chefs and dietary staff that prepare home made meals and snacks on site. Nutrition is an imperative part of good health.

Registered dietitians work with residents and their physicians to develop individual meal plans based on stated preferences and medical needs. A selective menu is offered to residents with choices within their prescribed diet.

Snacks and beverages are more than a pleasant treat. They are important supplements to insure caloric intake and hydration. Blossom View has dedicated dietary aides whose primary job is to offer and encourage periodic snacks and beverages throughout the day.

Residents that need the assistance of staff to complete their meal are served in several smaller dining areas. More independent residents eat in the Empire Dining Room or in Cameo's, renovated dining area open to the community.

Family and friends are welcome to join residents for meals any time. Advance reservations are helpful but not required. For birthdays, anniversaries or other special occasions, please let us know how we can help your family celebrate!


LEISURE AND RECREATION

Blossom View has acres of land including woods, an apple orchard and landscaped walk ways. (Click on the button to the right to see an aerial photo). In the middle of the facility is a landscaped courtyard with shade and sun.

All around the facility there are lounges and green corners for private time or socializing. These include the garden room, the billiards room, the computer room lounge, the craft room, and three additional general lounges with televisions and the main activity room. The chapel is open all the time. Services are held throughout the week.

Our activity staff keeps their calendar full of fun things to do with a variety that satisfies many tastes.

Activities also produces a monthly newsletter. It is distributed to all Residents and designated family members. It is chalk full of pictures stories, ideas and next month’s activity calendar.

Residents can go out and about to prearranged activities on our 12 passenger bus! Just sign up!

Want to get your hair done? We have a salon on the premises. How about a massage? It can be arranged!

Blossom View is pleased to introduce Diane Goergen, a Board Certified art therapist. Diane has joined the staff to begin an art therapy program in the facility. She graduated from Nazareth College with a Master's in art therapy in 1997. The new art therapy program is a blend of therapeutic recreation and clinical therapy.

"Art making can provide direct access to the emotional self through remembrances and experiences. Engaging in activities that are creative becomes the perfect tool for discovery and healing. Art materials provide a safe, often non-verbal and non-threatening form of communication." says Diane. Although most of the time, the resident is actively involved in art making, there are times when the resident's verbal story is "painted" by Diane. It is a way to put the resident's voice on canvas. She will work with individuals and groups.
