Adirondack Tri-County Nursing and Rehabilitation Center is an 82 bed skilled nursing home, nestled in the South Central Adirondack Mountains.
Opened in 1974 the facility has grown to include Physical Rehabilitation Therapy and a model Adult Day Care Service.
Our long tradition, 33 years, of providing quality care and maintaining the dignity of all our residents is of utmost importance. Our caring & well trained Nurses and CNA'S provide excellent care while maintaining the dignity and safety of our residents.

OUR ADULT DAY CARE CENTER
The Adult Day Care program at Adirondack Tri-County Nursing and Rehab.Ctr. offers a wide variety of services to meet the needs of all participants.
Working with each one's personal physician, we design a care routine that is appropriate for each individual.
Directed by a health-care professional these services include:
- Nursing assessment, supervision and monitoring
- Dietary consultation and monitoring
- Physical therapy and speech therapy
- Nutritional lunch and snacks
- Personal care services
- Counseling and information on community and social services
- Health education
- Religious services and counseling
- Recreation and activities such as arts and crafts, music, exercise, current events, games, field trips, movies, bingo and parties.
The Adult Day Care center is located in the nursing home in North Creek. Within a half-hour drive of Indian Lake, Warrensburg, Newcomb, Chestertown, Schroon Lake, Bakers Mills, and other communities. It is the only service of its kind offered in the South Central Adirondacks.
OUT PATIENT PHYSICAL THERAPY SERVICES
At Adirondack Tri-County we have a separate Out Patient Physical Therapy Facility.
We are the only Physical Therapy Service in the area and draw from Johnsburg and surrounding communities. We feel it is a community service we offer and are proud to serve the region.
Our Therapists are well trained and have experience in many areas of therapy.
We can evaluate and treat a wide range of conditions and diagnose including:

- Orthopedic conditions
- Sports Injuries
- Work Injuries
- Carpel Tunnel
- Multiple Sclerosis
- Parkinson's Disease
