Achieve Rehab and Nursing Facility located in Liberty, New York in beautiful Sullivan County is a long-term skilled nursing facility and a short-term subacute rehabilitation center featuring physical, occupational and speech therapy.
At Achieve Rehab and Nursing Facility, we provide outcome oriented treatment techniques, for both long and short-term residents. Our skilled interdisciplinary team, consisting of doctors, nurses, physical therapists, speech therapists, dieticians, social workers and recreation personnel, constantly reviews our residents' "plan of care" and monitors changing needs on an individual basis. Our Social Services Department assists with the transition to life at Achieve, for both long and short-term residents. When residents are ready for discharge, our Social Services Department works with various agencies to secure services providing for a pleasant transition back into the community.

SHORT TERM REHAB
Drawing upon years of hands-on experience and using a team approach, we provide outcome-oriented treatment techniques, focusing on quality care. Upon admission, each patient receives an initial evaluation to formulate an individualized program based on their needs. Our skilled interdisciplinary team constantly monitors and modifies the plan of care as appropriate for the patient's ever-changing needs.
Upon the patient's discharge from the facility, our social services department ensures that the continuum of care is followed so the patient reaches the highest level of functioning when returning to the community.
Our subacute care team is committed to helping patients maximize their potential through rehabilitative services. We help patients regain their independence so they can return home as soon as possible. Our emphasis on patient education and self-care assures lasting results. LONG TERM REHAB

Recreational activities are an integral part of our lives. Without them, we become withdrawn, lonely and depressed. The recreation department of Achieve Rehab and Nursing Facility provides a variety of diverse activities to allow residents to enhance and improve their quality of life. Our program incorporates activities for verbal, sensory, tactile, physical and cognitive stimulation. Residents are encouraged to actively participate in scheduled activities on the unit, but are provided with the opportunity and tools necessary to pursue independent leisure pursuits of interest and choice.
