Our Team Approach
Every TownHouse staff member is selected after careful screening to ensure appropriate education preparedness, experience and interpersonal skills. Thereafter, the Center's Educator gives additional training to advance caregivers' knowledge and expertise in incorporating wellness into the delivery of services.

Each resident’s team is comprised of clinical professionals in accordance with the resident’s needs. The resident and his/her designated family representative are welcomed members of the resident's health care team. This approach promotes communication, collaboration, and the establishment of mutually-agreed upon goals.

The Center is comprised of qualified personnel, top to bottom, from our New York State-licensed Administrator to trained and certified Nursing Assistants.

Professionalism in practice is a TownHouse expectation of every employee. We know that with a sense of professionalism comes job pride, followed by excellence in service.

Quality Assurance (QA)
In TownHouse, continued pursuit of excellence is not simply lip service, but a viable system. Through observation, interview and data review, the Center's Quality Assurance (QA) monitoring program conducts ongoing evaluation of care, conditions and services provided by all levels of personnel. Our QA approach includes resident and family satisfaction studies. The results of all monitoring and studies are analyzed by the QA Committee. When shortfalls or opportunities for improvement are identified, they are promptly addressed. As a result of the Center's QA program, we are pleased to report that we are in a continuous cycle of affirmed excellence.

Medical Services
We have an unwavering commitment to provide the highest quality medical care. Each resident has an Attending Physician. A Nurse Practitioner (NP) augments the medical model. The NP is an extension of the physician and is on-site each day to evaluate residents that have had a change in status. The NP collaborates closely with the Attending Physician. Maintaining communication with the resident/family is a part of our medical standard of practice. Other available medical services, as needed, include virtually all Medical Specialties and:

Nursing Services
Registered Professional Nurses (RN), Licensed Practical Nurses (LPN) and Certifed Nursing Assistants (CNA) provide care under the supervision of an RN Unit Manager, 24 hours per day, 7 days per week. On admission, each resident is assessed by an RN, and a plan of care is developed to meet the resident's nursing and wellness needs. This is done by (a) working with each resident's strengths, (b) providing rehabilitation nursing, (c) maintenance of the resident's highest, practicable, level of function and (d) treatment and/or prevention of active medical conditions.

Social Services
Dedicated Social Workers (SW) serve as resident advocates and liaisons. Each resident is assigned a SW who assesses and addresses social and emotional needs. The SW's advocacy role ensures that care is given to promote and enhance quality of life, dignity and respect. For short-stay residents, the SW facilitates the discharge plan and coordinates the team in attaining the resident's safe and successful return to his/her home.

Rehabilitation Services
The Center provides short and long-term intensive rehabilitation. The goal of rehabilitation is to assist residents to attain their highest, practicable level of independence and function. The Physical, Occupational and Speech Therapists are part of the care-planning and care-giving team.

Two fully-equipped gyms with state-of-the-art equipment complement the rehabilitation program. The treatment area for Occupational Therapy contains a working kitchen and bathroom that replicate a home setting. This design helps short-term residents gain the realistic daily-living skills needed to safely and successfully return home. Conditions treated by rehabilitation therapy include:

Theraputic Recreation Services
With a mind and eye toward wellness, the Recreational Department offers a vast array of meaningful therapeutic activities to all residents. The programs are facilitated by Recreational Therapists and designed to promote health and function through rewarding and enjoyable activities. From a focal point of wellness, programs are created to make residents:

Get Well: Through promotion of healthy mind and body activities, customized to the resident's capabilities.
Live Well: Through active socialization/involvement in daily events.
Stay Well: Through provision of programs that sustain each resident's highest level of function.

Nutritional Services
The Center prepares and serves quality foods, using the finest ingredients and cooking techniques. The Dietary staff understand the importance of awakening the senses of smell and taste, nourishing the body and promoting health and wellness. They do this through preparation of tantalizing meals, served in a pleasurable dining milieu. The Center's Dieticians are professional clinicians who assess nutritional needs and plan, implement and evaluate the effectiveness of special diets and alternate nutritional means. The Dietician is on the team that transitions weanable residents from feeding tubes.

Respiratory Therapy
The Center has a New York State-certified, 20-bed, ventilator-dependent unit that provides specialized care to persons on mechanical ventilation. The unit's Physician joins forces with a Pulmonologist and oversees the licensed Respiratory Therapist, NP and RN. All work together to assist the ventilator-dependent resident to attain full potential. Respiratory care is provided 24/7. The unit also serves persons with other complex respiratory conditions, including those in need of tracheostomy care, oxygen or inhalation therapy. The Center has achieved high weaning and rehabilitation rates in New York, returning over 70% of short-term, ventilator-dependent persons to their homes.
