Sunset Nursing and Rehabilitation Center, Inc. is a one hundred-twenty bed skilled nursing facility which provides twenty-four hour residential health services for persons who are no longer in an acute phase of illness but who continue to require frequent medical nursing observation, care and rehabilative services.

Our purpose is to help chronically ill or disabled residents to reach and maintain their greatest potential and to continue to enjoy a quality of living experience adapted to their individual health status and personal preferences. Thus, our goal is not only to promote physical health, but also to encourage functional ability in all ways: physically, emotionally, mentally and socially.

GENERAL SERVICES

Skilled Nursing Care

24 hour skilled nursing care provided by Registered Nurses, licensed Practical Nurses and Certified Nursing Assistants. All skilled Nursing services are provided under the direction and supervision of a Board-certified Medical Director with special training in geriatric medicine. Individualized skilled nursing services include:

Short Term Rehabilitation

Rehabilitation services are designed to assist residents in their recovery from surgery, acute illness or injury. Rehabilitative care teams includes nurses, occupational therapists, speech therapists, dieticians and a social worker. All therapy programs are individually developed based on condition and goals. In addition to intensive Occupational and Speech therapy, Rehabilitation services include:

> Post - surgical care

> Tracheostomy Care


Respite Care

Respite stays are flexible and may range in length from several days to a month. Respite care services include 24-hour skilled nursing and rehabilitation services. All respite care services are under the direction and supervision of a Board-certified Medical Director.


Out-Patient Occupational Therapy

Outpatient therapy services are provided to those recovering from surgical, acute illness or injury. Outpatient therapy is conducted under the order and supervision of a physician. Services are provided by Registered and licensed Occupational Therapist. INDIVIDUALIZED RESIDENT SERVICES

- Therapy
Occupational therapy, Speech tharapy and Physical therapies are provided to the residents of Sunset as prescribed by their attending physician. Therapy is provided under the direction of a Registered Therapist.

- Dental
A dentist is available to provide routine and emergency dental care.

- Podiatrist
A qualified podiatrist is available for services upon order of a personal physician.

- Beautician and Barber
A licensed beautician is on the premises at least one day each week. Appointments may be made through the receptionist or Nurse Manager. A member of the resident's family or another licensed salon beautician may provide beautician services for the resident and use the salon facilities. Beautician services are the financial responsibility of the resident, family or responsible party.
Shampooing of the hair is a regular patient care activity. Nursing staff may set hair if they wish to, but they are not required to do so and receive no monetary compensation. Cutting, permanents, etc. must be performed by the appropriate licensee.

A licensed barber is in regular attendance at the nursing home. As above, the fees for the services of the barber are the responsibility of the resident, family or responsible party and are not included in the daily rate.

- Laundry
The skilled nursing facility will launder underclothing, nightgowns, pajamas and other machine - washable articles without charge, but will assume no responsibility for the intactness of these articles. Families or other responsible parties are requested to attend to the articles of clothing requiring hand laundering or dry cleaning. If they prefer to launder all personal clothing, then family may make such arrangements with the Nurse in Charge. It is expected that residents and their families will plan the replacement of clothing and other personal articles with the nursing staff.
Clothing will be labeled by the facility with his or her full name either by sewn-on or iron-on name tape or laundry pen. Washable clothing is requested. It is expected that family or other responsible party will have sufficient numbers of articles which are comfortable, modest and appropriate to the resident's condition, needs and preferences.

- Toilet and other Personal Articles
Articles used in regular oral hygiene and skin care are provided by the facility. If a resident prefers a different brand or type of item than that provided by the facility or if the attending physician recommends a medicated or other special type of item, then it is the responsibility of the resident or responsible party to provide such an item. Cosmetic items are not provided by the facility.

- Eyeglasses, Dentures, Hearing Aids and other Appliances
The staff of the facility take every reasonable precaution to safeguard eyeglasses, dentures, and hearing aids but cannot be responsible for their breakage or loss. Families may want to leave extra glasses, batteries for hearing aids, etc. with the Nurse in Charge. When visiting, families or responsible parties should check periodically to ascertain whether or not replacement articles are needed. The obtaining, repair, and re-examination for such articles are the responsibility of the resident, family or responsible parties.

- Patient Incidental Account, Money and Jewelry
Prudence dictates that residents not have cash in their rooms or on their persons. A Patient Incidental Account of either interest-bearing or regular type are available for residents and responsible parties who wish to leave money at the facility. Such an account may be opened with the Receptionist and must maintain a minimum balance of twenty dollars. A ledger is kept of each resident's account transactions and a quarterly accounting is provided to each resident and/or responsible party or sponsoring agent. Residents and their families are encouraged to avail themselves of this service from the day of admission since experience has shown that incidental needs do arise. If families wish to leave money for the account during weekend visits, please see the Nurse in Charge. We advise, however, that such business be conducted during regular business hours or by mail in consideration of the nursing staff's primary care responsibilities.
The Sunset Nursing and Rehabilitation Center cannot be responsible for money or other valuable items. We suggest that residents not bring jewelry or other valuable keepsakes. If, however, they must be brought to the facility, then they may be placed in Valuables Envelopes in the Social Service Office. Residents are welcome to wear wedding rings and watches unless prohibited by their physical condition. Families are requested to advise the Nurse in Charge when removing any belongings of the resident from the facility.

- Newspaper
Arrangement for a daily newspaper may be made with the receptionist

- Mail & E-mail
Resident's mail is delivered to the Nursing Home daily via the United States Postal Service. Letters and packages should be addressed as follows:

- Shopping Service
Twice each year, the organization known as Residents' Shoppers Service sets up their extensive collection of affordable fashions in the Activities Room so as to enable our residents to shop for new clothes and fashions in the comfort of their own home.
