Welcome!
We understand that placing a loved one in a nursing facility can be a difficult decision for relatives of the elderly. Whether it be for long term care or short term rehabilitation, families seek a facility which provides excellent care in a warm and loving environment.
At Port Chester Nursing and Rehabilitation Centre, one of our priorities has been to develop a therapeutic, nurturing and supportive environment for residents. We have found this to be very effective in motivating residents and facilitating recovery.
Our home-like 160-bed facility offers quality long-term care, short-term rehabilitation and subacute care, as well as many amenities designed for our residents' comfort. Our newly opened Coffee Shoppe is a magnet for residents and family members alike, where they can enjoy freshly brewed coffee accompanied with mouthwatering home-baked pastries. WOUND CARE
Millions of Americans suffer from chronic wounds caused by diabetes, vascular disease and other conditions, and these hard-to-heal wounds can only get better with constant medical intervention. Port Chester Nursing and Rehabilitation Centre is at the forefront of comprehensive wound care, providing residents with the most advanced wound care technologies coupled with attentive and compassionate care
Our dedicated wound care team has scored impressive achievements in the treatment of Stage 1-4 wounds, developing specific care plans for each patient while offering them the best quality of life possible in an environment of caring.
Our medical director, wound care nurses, infection control nurse, physical and occupational therapists and a dietitian comprise our skilled and experienced team. With strong community ties, our affiliated physicians in plastic surgery, general surgery, dermatology, podiatry, vascular and infectious disease provide consultation and service.
Upon admission for wound care, patients are evaluated by the wound care team to assess the wound for size, granulation, odor and drainage. Our multi-disciplinary approach includes the use of IV antibiotics, specialized air mattresses, infection control techniques and state-of-the-art equipment and devices, such as the negative pressure vacuum-assisted wound closure for reduced healing time and pain management. Pain management is a primary focus when treating wounds.
In addition to a focus on wound healing, we treat the "whole" individual, offering physical and occupational therapy, recreation programs and activities and healthy, balanced meals served three times a day in our beautiful dining room. Because nutrition plays such a key role in the healing process, our registered dietitian provides counseling and patient/family education on healthy and nutritious eating and also customizes a diet for each resident.
REHAB
We offer comprehensive post-operative rehabilitation services.
Our high-caliber rehab team is comprised of therapists with years of experience. Subacute residents spend the bulk of their day in rehab  with therapy provided 2-3 times a day, 6 days a week. We can even provide therapy over the weekend if necessary.
We understand that each day after surgery is one step closer to a full, functional recovery and every resident’s goal is reassessed regularly. The crafting of each individual’s rehab regimen is an active process determined in part by the resident’s input.
