Owned and operated by Washington County, Pleasant Valley is a recognized leader in the management of long term health care and services to Senior Citizens.

Pleasant Valley is home to 153 residents who are in need of varying degrees and types of nursing care. Established in 1827, Pleasant Valley has a long standing tradition of providing quality and compassionate care to area residents, in the setting of Nursing Home, Adult Home or Adult Medical Day Care. We are in partnership with Glens Falls Hospital to provide Physical and Occupational Therapy to both our residents and in the near future, community residents on an out-patient basis


NURSING HOME

Pleasant Valley has 122 beds dedicated to residents with varying medical needs. Forty of these beds are on our Alzheimer’s Unit.

Alzheimer’s Unit
In order to better meet the needs of our residents, Pleasant Valley opened its state of the art Alzheimer's Unit in 1999. This discrete unit offers many design features which were introduced based on the unique needs of individuals afflicted with Alzheimer's Disease and other forms of dementia. A tremendous amount of research and planning went into the design of this unit which has made the unit a tremendous success. Facilities from all regions of the State have come to Pleasant Valley to tour this unit.

"Double Loaded" hallways allow residents to walk freely throughout the unit while never coming to the end of a hallway. All hallways have continuous loops which minimizes a resident’s level of frustration. We also designed a secure outdoor courtyard adjacent to the unit. Residents may walk on the paved pathways next to the building in a secure courtyard where they may be protected from traffic. Benches are provided at regular intervals along the pathways to afford the residents the opportunity to rest and simply relax and enjoy our country setting.

Also, the unit has many small congregate areas. Research has shown that residents suffering from Alzheimer's Disease and other similar afflictions are much happier and more comfortable in smaller groups. Four resident households as well as an ice cream parlor and a quiet room offer our residents several options as to where they would like to be. All of these areas offer direct staff contact or observation.

The unit is also very bright. There are many large windows as well as skylights to keep the unit bright and cheerful. This has also been a proven strategy in managing individuals with Alzheimer's Disease.

This unit has been a tremendous tool in allowing us to fulfill our mission of providing quality services to our residents while promoting their independence, choice and dignity.

Traditional Medical Units
Pleasant Valley’s two original nursing units are Mid Town and Hill View. Mid Town is a 42 bed unit while Hill View has 40 beds. These two units, which provide for traditional geriatric medical care, were opened in April of 1970. Mid Town and Hill View were fully renovated in 2002 which completed a master renovation program which updated every resident room and resident area in the building. All resident rooms are cable and internet ready.

Most skilled nursing care is provided to the aged with significant chronic afflictions. Our nursing units provide for virtually all of the medical needs of our residents. For those needs that can not be met in house, we arrange for transportation to the appropriate care provider.

We provide for regularly scheduled on-site physician visits as well as 24 hour access to a physician. Being a smaller rural facility allows our medical director and his associates to become very familiar with all of our residents and their specific needs.

A dentist is on site every week to provide routine visits as well as address any urgent needs. We also provide for regularly scheduled on-site visits by area podiatrists, urologists and optometrists.

We provide for around the clock nursing services provided by a team of registered nurses, licensed practical nurses and certified nursing assistants. We pride ourselves on the personal relationships that we build, not only with our residents, but their families.

Pleasant Valley is very pleased to have an affiliation with Glens Falls Hospital for the provision of rehabilitation services. Glens Falls Hospital is regarded as an industry leader in the provision of restorative therapy services.

We have committed a large amount of our resources to investing in our physical plant. We have an incredible building and setting. However, our greatest asset is our dedicated staff. Their work ethic and genuine compassion for our residents is the foundation of all that we do, it is what sets us apart.


REHABILITATION SERVICES

Pleasant Valley works hard with each of our residents to assist them in reaching and maintaining their highest possible level of function. To that end, Pleasant Valley has developed an affiliation with Glens Falls Hospital for the provision of Physical Therapy and Occupational Therapy services. Glens Falls Hospital's success in providing state-of-the-art rehabilitation and returning people to their home settings is critical to helping patients improve the quality of their lives.
In partnership with Glens Falls Hospital, Pleasant Valley offers exceptional rehabilitation therapy services to meet the physical needs of residents of the skilled nursing facility. We are committed to meeting the long-term care needs of residents, as well as assisting people to return to their home settings whenever possible.

Occupational therapy (OT) provides evaluation and treatment to enhance a person's ability to perform self-care and other daily tasks with greater ease.

OT teaches compensatory techniques concerning self-feeding, dressing, bathing, grooming and higher level household management.

Physical therapy (PT) provides evaluation and treatment to enhance a person's ability to increase strength and mobility.

PT increases a person's strength and range of motion. PT also teaches compensatory techniques to accommodate a person's deficits relating to walking and transferring (for example, from a wheelchair to a bed, from a wheelchair to the bathtub, etc.).

Additional Services

In addition to standard therapy disciplines, the Rehabilitation Department of Pleasant Valley offers a variety of other services such a:

- Joint OT-PT home visits to assess a person's home needs
- Other medical services offered through Glens Falls Hospital

Speech Therapy
Pleasant Valley also offers certified speech therapist services. Speech therapy assists people in regaining speech skills that may have been lost or diminished due to a stroke or other event. Speech therapy also assist individuals with eating deficits and works closely with our food service department in planning for the most appropriate diet based on resident specific needs.


OTHER SERVICES

Other Services available to Pleasant Valley residents or Medical Adult Day Care registrants:

- Nursing Management: An RN is on duty during program hours to provide and/or supervise nursing services.
- Personal Care: Bathing, Century Tub Hydrotherapy, dressing, grooming and toileting, including bowel/bladder retraining programs.
- Treatments: Skin care, foot care, dressings, wound care, catheter changes, irrigations, ambulation, feeding and injections.
- Healthcare Monitoring: Vital signs, blood sugars and cultures.
- Medications: Supervision, assistance and evaluation of effectiveness
- Therapeutic Activities Program: Based on client goals and needs, includes: music, religious services, exercises, games and socialization in an enjoyable environment.
- Social Worker: To provide advocacy services.
- Dental and Podiatry Services
- Diagnostic Services - Laboratory testing and some X-rays, EKG.
- Beautician/Barber Services-by appointment for a separate fee.
- Nutrition Services - Balanced nutritional meals and snacks are provided, and nutritional consultations are available by physician’s order.
