Our Lady of Mercy Life Center is committed to providing quality services with the outstanding compassion for which St. Peter's is renowned. Our well-trained staff's devotion to our residents leads to better outcomes and higher ratings in resident and family satisfaction surveys.

Our Center's location in Guilderland, NY, is a tranquil, wooded setting with easy access from one of the region's major roadways, Route 20. This tranquil setting features a private courtyard, a waterscape area, beautiful plant life, and a creative herbal garden in which residents are encouraged to participate. Resident activities take place in the courtyard, weather permitting, including A Summer Concert Series that is enjoyed by both residents and their families.

Divided into four units of 40 residents each, four private rooms and 18 semiprivate rooms, the Life Center strives to maintain the intimacy of a homelike setting. To help foster a sense of community and friendship among the residents, each unit has its own distinctively decorated dining room.

We are proud to be recognized by New York State's Island Peer Review Organization (IPRO), a national leader in health care evaluation and quality improvement, and the NYS Department of Health Best Practices Programs for our work in a number of clinical areas. In addition, we are recognized in the HANYS Pinnacle Awards Publication, and as a lead agency on numerous NYS Departments of Health and Labor grants. Our Lady of Mercy Life Center staff has presented its best practices at a number of professional conferences.


Subacute Care/Rehabilitation

Upon admission, each resident is evaluated for an individualized program of occupational, physical, speech and/or audiology therapy. The program is carefully monitored to help residents achieve the most satisfying level of independence. Our goal is to enable and enhance each resident's abilities to enjoy an active, meaningful and fulfilling life.

Services provided include:
- physical therapy;
- occupational therapy;
- skilled nursing rehabilitation;
- speech therapy and audiology;
- exercise group; and,
- evaluation for any equipment needs.


Subacute Care Rehabilitation

Our Lady of Mercy Life Center's Rehabilitation Program provides a broad range of professional services designed to meet patient needs. For example, when an individual cannot tolerate or does not require a hospital-based acute rehabilitation program, a subacute program at Our Lady of Mercy Life Center is a great place to begin the rehabilitation process. Whether the length of stay at the Center is several days or several weeks, a personalized plan of care assures appropriate medical, therapeutic and social services and discharge planning at every stage of the rehabilitation process. Our goal is to help patients reach an optimal level of function and return home safely.

- Dedicated rehab section
- Rehab patients admitted to care of physiatrist as primary physician
- Rehab Department staffed by our employees, providing two or more hours of combined therapy on a daily basis in a six-day program with two physical therapy sessions per day as ordered
- Dedicated registered nurse discharge planner
- 33-day average length of stay
- Beds often readily available with a rapid resident turnover
- Wound VAC (vacuum-assisted closure) negative pressure wound therapy
- IV therapy
- Bariatric patients accommodated


Alzheimer’s/Dementia Unit

The Guadalupe Unit specializes in the care and treatment of persons with dementia. Guadalupe's focus is on providing our residents with opportunities that will preserve the highest level of function while meeting their individual cognitive, physical and spiritual needs.

Through the use of the Joy of Life program, our residents are offered activities and social opportunities, streamlined to their specific abilities and preferences. We embrace a multidisciplinary approach to care for our residents upon admission and as they age in place. Under the leadership of professional registered nurses, our staff is knowledgeable about the unique challenges of caring for persons with dementia and brings sensitivity and compassion to their work consistent with the mission of St. Peter's Health Care Services.


Activities

We offer a variety of social and therapeutic activities on a daily basis. The Activities Department consists of a certified therapeutic recreation specialist and six activities assistants.

Special programming is provided for Alzheimer's/dementia residents. The department coordinates special entertainment series and resident outings on a regular basis.
