Established in 1832, the Horace Nye Home in Elizabethtown, New York, has long been recognized for its excellent service to the residents of Essex County. Home to approximately 100 residents who require skilled and/or personal long-term and short-term care services, the facility offers private and shared accommodations with telephone and cable hook-up, a large and spacious dining room, and a lovely, centrally-located and landscaped courtyard. Located behind the Essex County Government Center, and adjacent to the Elizabethtown Community Hospital and 24-hour emergency services, the Horace Nye Home affords a central community setting with proximity to essential clinical and social services.

Horace Nye Home provides 24-hour skilled nursing care in a safe, secure, and home-like environment in a smoke-free facility. Each resident receives individualized assessments to develop a plan of care best suited to attain and maintain his or her highest possible level of functioning, with emphasis on independence and choice.

Residents are admitted from a variety of settings including acute care hospitals, non-skilled facility settings, home, or other skilled nursing facilities.

The Horace Nye Home prides itself in its involvement in the community. We welcome individual or group participation in our facility, including daycare groups, musicians, youth or club groups, and recitals and plays performed by local talent. The facility's handicap-accessible vans offer our residents the opportunity to explore and enjoy activities in Essex County and beyond.

Medicare and Medicaid certified*, the Horace Nye Home offers a full array of services including skilled nursing and health-related care; restorative and maintenance physical therapy; social services; leisure time (view the June calendar, requires the free Adobe Acrobat Reader), dietary and housekeeping services; dental care; and access to speech, podiatric, audiologic, and eye care. Cost of care is currently $250.00 per day (effective 1/1/2009). In the fiscal year from January 1, 2009 through December 31, 2009 Horace Nye Home will provide uncompensated services to all eligible persons unable to pay who request those services. All services of the facility (except those services covered by a third-party or governmental program, such as Medicare and Medicaid; and the patient resource amount which Medicaid has determined the resident must contribute to the continuing cost of care) will be available as uncompensated services to persons whose family income is not more than three times the Poverty Income Guidelines (known as Categories A, B, and C). The Poverty Income Guidelines are established by the U.S. Department of Health and Human Services. This notice is published in accordance with 42 CFR 124.505, Notice of Availability of Uncompensated Services. We invite interested parties to comment on this allocation plan.
