Heritage Green Rehab & Skilled Nursing is a 151-bed, long-term and rehabilitative care facility located just minutes from Jamestown in Greenhurst, New York. "The Green" is nestled in a beautiful area of Chautauqua County and is only a short walk from the tourist haven, Chautauqua Lake.

In 2002, Heritage Green completed an extensive contruction and renovation project, giving the facility enhanced accommodations and more spacious common areas for residents to enjoy. The renovations added 51 beds (Heritage Green previously had 100 beds), a chapel/prayer room, kitchen, dining room, occupational & physical therapy suites and a multimedia auditorium.

In addition to the completed renovations, Heritage Green is also looking forward to a new heating, air conditioning and ventilation system.
In the fall of 2003, Heritage Green also installed an innovative outdoor sensory garden, Tranquility Garden. This garden is specifically tailored to the needs of residents suffering from the effects of Alzheimer's Disease and dementia.

The team approach at "The Green" enables staff to offer residents the best professional care in a loving, home-like environment. The primary focus of all staff members is on the individual resident's needs and the preservation of his or her dignity and respect. In response to the growing need, Heritage Green recently began offering peritoneal dialysis at the facility, allowing residents in need of this service to remain "at home." Every resident is continually monitored by members of the nursing, social work, dietary, and therapy departments. Heritage Green views care in a holistic manner, catering to the physical, emotional and spiritual needs of every resident.
