Ferncliff Nursing Home is served by the Carmelite Sisters for the Aged and Infirm, a religious community nationally renowned for their compassionate care for the elderly.
The Sisters’ compassion is shared by the Home’s professional and support staff, who provide the best possible experience for residents and their families through holistic care -- addressing not only physical needs, but social, psychological, emotional and spiritual needs as well.
A wide variety of recreational opportunities are available to the residents, and there are special programs designed for those who are bed-bound. In addition to excellent long-term care and short-term rehabilitation programs, Ferncliff also offers an Adult Day Health Care program to those needing assistance maintaining their independence.
Ferncliff is located in the Hudson Valley community of Rhinebeck, on 36 beautiful acres of the former Astor family country estate. Views from each of Ferncliff’s 328 private or semi-private rooms reveal meticulously maintained grounds with acres of verdant lawns and mature stands of trees.
While the landscape is very impressive, first-time visitors to Ferncliff are often most impressed by the staff’s true spirit of hospitality and the tremendous enthusiasm they show for their work.
SKILLED NURSING CARE
At Ferncliff Nursing Home we offer warmth and compassion in a setting that optimizes dignity and respect. We are committed to providing the highest quality skilled nursing care by the best-trained professional staff in a homelike, safe, and inviting environment.
To determine service needs, each new resident receives a comprehensive evaluation. The resident's physician—experienced in the care of the elderly—coordinates an individualized care plan and leads a multidisciplinary team including licensed nurses, therapists, and dietitians.
With input from the resident and the resident's family, goals are established for achieving and maintaining maximum independence. The resident's progress is reviewed regularly, and care plans revised as needed. Full time recreation therapists and social workers, as well as a Pastoral Care professional, respond to recreational, psychosocial and spiritual needs.
Our primary goal is to treat each resident as an individual and to support the highest possible level of function.
REHABILITATION
Whether a person is recovering from an illness, surgery or accident, Ferncliff Nursing Home offers a full range of rehabilitative programs that help short-term residents regain their health and as much independence as possible.
Working together with their patients, our dedicated team of doctors, nurses, therapists and social workers tailor individual programs that help residents regain important life skills and abilities to the fullest extent possible.
We use the latest techniques and treatments to maximize effective outcomes. Therapy sessions are scheduled throughout the day to promote faster recovery and to speed the journey home.
Therapists provide care and support to help residents reach their rehabilitation goals. Our aim is to have residents return home safely – with strength and confidence in their ability to maintain their independence for as long as possible.
ADULT DAY HEALTH CARE
For those having difficulty structuring their daily activities, feeling isolated, needing companionship, or unable to be safely home alone, Ferncliff’s Adult Day Health Care offers a practical solution.
Adult day health care is a progressive, cost-efficient way for aging individuals to stay connected to their homes, families, friends and communities. Comprehensive programs provide health, rehabilitative and therapeutic services while fostering independence and strengthening physical and emotional wellbeing.
Through adult day health care, participants also have opportunities for social interaction and sharing in stimulating activities. Our programs focus on enriching participants' lives, while helping them remain engaged and active in their community.
The program benefits family caregivers too, by giving them a much-needed break from the physical and emotional demands of providing care, and the reassurance that their loved ones are in good hands.
