Evergreen Commons was created to offer an enlightened approach to providing health care services to the elderly. Our philosophy is to ensure that residents have skilled and compassionate care while enabling them to experience the highest quality of life. The Evergreen Commons mission is to provide a place where the elderly are treated with dignity and respect, and where activities are planned that support each person’s independence and self-esteem. We do all we can to create an environment that allows our residents to enjoy this new phase of their lives, feel they are part of a caring family, and know they are safe and well looked after by a concerned and capable staff.

Facilities

Evergreen Commons is a new style elder care facility designed to enable residents to enjoy their later years in a comfortable setting that supports maximum independence and safety. Many first time visitors are pleasantly surprised by the facility’s warm, homelike ambience and numerous amenities. The healthcare needs of each resident are attended to with a comprehensive range of medical and rehabilitation services including 24-hour skilled nursing care when necessary. Along with quality health care, we are equally committed to seeing that residents experience the best possible quality of life. We offer bright, cheerful rooms, restaurant style dining, individual and group activities, recreational activities, beauty parlor/barber shop services, plus housekeeping, laundry and other services that help foster the dignity and autonomy residents cherish.

Rooms

The comfort of every resident was a prime concern in the design of Evergreen Commons. We have created private and semi-private rooms that are beautifully furnished and offer all the amenities for comfortable, secure living. Each spacious room features a bay window with a scenic view, individual temperature controls and its own bathroom. Residents are encouraged to bring familiar and cherished items to give their rooms the personal touches that enhance the inviting, home-like feeling.

Food Service

Dining together as a family is one of the most pleasurable social activities our residents look forward to each day. Our professional foodservice staff takes great pride in preparing meals that are both delicious and nutritious. We strive to provide a fine dining experience while creating meals guided by each individual’s special dietary needs or restrictions. Meals are graciously served in our attractive and comfortable dining rooms. Room service is also available upon request or when needed. And, with short notice, family members can join their loved one for breakfast, lunch or dinner.

Activities

At Evergreen Commons, it is our goal to provide not only the highest-quality care, but to offer each resident the opportunity to continue leading the same, full active lifestyle he or she did at home. That’s why activities with family and the community are the everyday heartbeat of Evergreen Commons. Our Recreation Department plans a full roster of weekly, monthly and annual events that keep residents engaged. From holiday festivities to birthday parties, from picnics to attending ballgames, residents enjoy a wide range of activities that keep them mentally and physically stimulated, while providing important social interaction and just plain fun.


HEALTH CARE SERVICES

Evergreen Commons offers a range of health care services including comprehensive long-term care, short-term rehabilitation, post-hospital care, a secure dementia unit and respite care. Evergreen Commons is one of the few senior care facilities to have a full-time, in-house Medical Director to oversee our medical and professional staff. Our doctors, physician assistants and nurses provide individualized care to meet the specific physical and emotional needs of each resident. Plus, local dentists, podiatrists, psychologists and optometrists make regular visits.

SHORT-TERM REHABILITATION. The goal of the rehab program at Evergreen Commons is to help residents achieve the skills they need so they can return home quickly and safely. We provide rehabilitation services for patients recovering from joint replacement surgery, a stroke or a prolonged hospital stay. We can also provide wound care, trach care, and IV therapy. Our physical, occupational and speech therapists are experienced in geriatric rehabilitation, carefully evaluate each resident and provide a program of individualized care. Family members are welcome to attend therapy sessions to make the transition home easier. We can also set up a home evaluation to ensure the home environment is safe and arrange for delivery of any necessary equipment or services.

POST-HOSPITAL CARE. In this era of shorter hospital stays, it is more important than ever to have a highly skilled, local facility that can assist patients through the recovery process. Evergreen Commons’ experienced and diverse staff of medical professionals ensures that patients receive the highest level of care. Whether for surgery, stroke, debilitating illness, fractures, joint replacement, or other needs, our well-informed doctors and nurses provide an individualized patient care plan that meets the specific physical and emotional needs of each resident. With a full range of health care services, the support of our dedicated staff, plus the latest technology and state-of-the-art medical equipment, Evergreen Commons provides an environment of comfort and healing that helps ease the transition from hospital back to home and community.

COMPASSIONATE AND SKILLED LONG-TERM CARE. One of the most important reasons families choose Evergreen Commons is the experience, skill and sensitivity our medical and support staff have in working with the elderly. We are proud to have a team of knowledgeable and dedicated people who provide the support and understanding needed to help residents and their families cope with the changes they are facing. We are committed to enhancing our residents’ quality of life with compassionate and attentive one-on-one care that meets each person’s medical, physical and emotional needs. We have created an environment of “small neighborhoods” throughout the facility to foster a feeling of comfort and security, and we encourage participation and socialization through family-style dining and numerous group activities.

A SPECIAL UNIT FOR A DEMENTIA CARE. For residents with a diagnosis of Alzheimer’s or other related dementia problems, Evergreen Commons offers high-quality care and expert supervision. A Special Care Neighborhood, devoted solely to residents with memory impairment problems, provides a supportive and secure environment with a focus on activities that benefit the residents who live there. An interdisciplinary team of doctors, nurses and therapists assesses each resident’s abilities and develops a care plan that reflects the unique needs of that individual. Interaction with other residents and staff is encouraged and monitored at all times. A tour of the dementia unit can be arranged at your convenience.

RESPITE - CARE FOR THE CAREGIVER. Whether it’s for other obligations, or simply to take a much needed break, there are any number of reasons at-home caregivers need “time off” from the intensive demands of caring for an elderly loved one. Our Respite Care provides the temporary support and relief you need, and gives you peace of mind knowing that your loved one is well cared for while you are away. Respite services include 24-hour nursing care, with medical, dietary, laundry and housekeeping services. Enjoying quality meals, meeting new friends, scheduling some time at the in-house beauty salon and many other activities keep short-term residents involved and their spirits high. And Respite enables caregivers to return with renewed energy and a fresher outlook.
