Eddy Heritage House Nursing and Rehabilitation Center is a 122-bed skilled nursing care center located in Troy, NY. Situated on top of upper Tibbits Avenue, the center overlooks the great Hudson Valley. Dedicated to enhancing the dignity, comfort and quality of life for seniors in a homelike environment, Eddy Heritage House has been providing quality care since 1992.


Skilled Nursing Care

The Eddy Heritage House campus features a beautiful and inviting courtyard right on the grounds, along with a comfortable, shaded sitting area for relaxing outdoors. For the many residents who enjoy seasonal gardening, we've also constructed a wheelchair-accessible patio garden with elevated planters to encourage those with a green thumb to participate.

Each of our resident floors has its own dining room, along with pleasant lounges for visiting and socializing throughout the center. The center also offers a variety of fun and interesting activities to stimulate residents, including music and pet therapy, and intergenerational programs with area schools.


Rehabilitation Services

Eddy Heritage House also provides short-term, sub-acute rehabilitation services for individuals who do not require the intensive rehabilitation therapy provided at an acute rehabilitation hospital. We proudly offer:

• a skilled rehabilitation team, led by a board-certified physiatrist (a physician who specializes in physical medicine and rehabilitation)
• a personalized therapy plan to help patients achieve the highest possible level of strength and mobility
• a dedicated rehabilitation unit
• therapy services available six days/week
• a registered nurse discharge planner to help with the transition to home.

The Eddy is affiliated with Sunnyview Rehabilitation Hospital in Schenectady.


Respite Services

For individuals normally cared for at home, Eddy Heritage House also offers respite care. Respite care gives family members and caregivers time to tend to other family or work obligations and relief from the grueling day-to-day caregiving responsibilities. Respite care is available at Eddy Heritage House one to four weeks at a time, with a maximum of six weeks of care during a 12-month period.
