The Cortland Care Center has been caring for members of our community since 1966. As we celebrate our 43rd year of service, we are proud of our long standing experience and of our reputation of being a quality healthcare leader in our community. We honor the people we have served.
Each bedroom directly adjoins a handicapped accessible bathroom. At present, we house dedicated care units for short-term rehabilitation, dementia care as well as long term care within six different care areas located in our health care center.
Our mission is excellence. We believe that our guests deserve a safe, attractive and comfortable residence where they receive the care they need. We take pride in the personalized and compassionate care we provide. An extensive variety of services, programs and staff members are provided to encourage and assist our guests to reach their fullest potential. We take seriously the expectations, goals and wishes of our guests and their families, which have resulted in consistent “high marks†in customer satisfaction.

The Cortland Care Center is approved and licensed for both Medicare and Medicaid programs.
REHABILITATION
What is “Short Stay Rehabilitation�
Short Stay rehab is designed to assist people who either are; in the hospital, or who have returned home from the hospital but need more time to recuperate. Reservations may be made in advance when scheduling surgery just prior to leaving the hospital.
You can also make reservations after you return home from the hospital and realize that you may need more help.
Maplewood – Short Term Rehabilitation Neighborhood
The Cortland Care Center began helping individuals return home from our health care center in 1990. Today, Maplewood, the 15-bed short-term rehabilitation unit, has assisted many community members in meeting their rehabilitative goals and subsequent discharge from our health care center.
Maplewood provides experienced rehabilitative care for most clinical needs including but not limited to: orthopedic, joint replacement, amputations, strokes, cardiac and wound care.
Therapy Staff
Cortland Care Center employs the most experienced team of physical and occupational therapists in our community. We have a team comprised of Physical Therapists, Physical Therapy Assistant, Certified Occupational Therapist and a Speech Therapist.
Maplewood Neighborhood Care Team
Our short stay rehab unit clinical team provides care under the direction of an attending physician and consultanting Psychiatrist. The Medical Staff and the nurse practitioner are onsite daily Monday-Friday. Our team includes RN's with extensive hospital experience, Wound Care RN, experienced LPN's and Certified Nursing Assistants, a case manager, a qualified dietician, and qualified Activity staff.
If you or your loved one need more time to recuperate after a hospital stay consider Cortland Care Center. We accept admissions from the local hospital. Referrals from other areas are also easily accommodated.
RECREATION
At the Cortland Care Center each day is alive with a variety of social events and festivities. Under the leadership of the Activity Director, our event’s calendar is full of meaningful programs. Our Activity Leaders provide social, therapeutic and entertainment programs. Residents enjoy the companionship of children, teens, plants and pets under the Eden-like environment. Our Residents look forward to annual seasonal events, the summer picnic and the holiday vocal concerts. Local churches provide religious support via weekly services and individual visits.
The Resident Council at the Cortland Care Center is very active. It has elected officers, holds monthly business meetings, makes recommendation for programs and operational improvements, and conducts fundraisers for community charity programs. Residents continue to demonstrate that they still have a lot to offer.
A dedicated team of 43 individuals, 7 groups and a number of school volunteers augment the activities.
