Welcome to the Chautauqua County Home. Chautauqua County Home is Western New York's premier long term health care facility. With a 216 bed facility in Dunkirk, New York, our rural setting and our staff provides a home-like atmosphere for our residents. The 280 member staff provides around-the-clock medical care as well as physical, occupational and speech therapy.

The Chautauqua County Home is a 216-bed skilled nursing facility, fully licensed by the State of New York Department of Health, and is certified by Medicare and Medicaid. We offer 24-hour professional nursing care to all residents. Residents and their families don't have to arrange for time-consuming, off-site visits for most medical services. Medical care includes our resident physician and dentist, and in-house lab, X-ray, podiatrist and optometrist. The Chautauqua County Home is rated as "the cleanest in the county." This is a very important factor when you select a facility for a family member.


NURSING HOME SERVICES

The Chautauqua County Home provides extensive skilled nursing home services.
Identified below are some of the services we provide to our residents:

Below are descriptions of other sevices we provide:

PHYSICIAN SERVICES

Upon admission the staff physician will be the Resident’s Attending Physician. The Attending Physician will visit at least once every thirty (30) days for the first ninety (90) days after admission and at least once every other sixty (60) days thereafter and more often when medically indicated.

The Chautauqua County Home contracts with medical professionals for most services. All contracted professionals are licensed, have a current registration certificate, and certificate of insurance. Residents retain the right to choose their own fee-for-service providers, however, if the Resident chooses to use a consultant not affiliated with the Chautauqua County Home the Resident must obtain prior approval from the Attending Physician.

RELIGIOUS SERVICES

Religious services are held on a regular basis. Any Resident who wishes to speak with his minister, rabbi or priest may ask a relative, friend or staff member to contact his clergy and upon request, the Activities Department will arrange for clergy of the Resident’s choice to visit. Roman Catholic and Protestant Chaplains are available for Residents and families. Protestant services are offered Sunday and Roman Catholic services are offered Wednesday. All Residents are welcome to attend any service.

In addition to church service, Rosary group and Bible study are scheduled by the Activities Department.

AUXILIARY

The Chautauqua County Home Auxiliary was formed in 1984 and has conducted numerous projects to enhance the quality of life for our Residents. It is through the efforts of the Auxiliary that certain projects and improvements have been undertaken. One such outstanding project was the construction of a patio for the enjoyment of Residents and their visitors. In addition the Auxiliary has had a new speaker system installed, large screen stereo with surround sound, clocks in all rooms, summer swing for handicapped persons, renovation of the gazebo for year round use, companion radio and redecorating the Day Room.

Volunteers groups and individuals provide the vital link between the Chautauqua County Home and the community. People of all ages share their time and talents to make life more worthwhile for our Residents. If you, or someone you know, would be interested in assisting at meal times, visiting or sharing skills or hobbies, please contact the Activities Department. A Chautauqua County Home Auxiliary membership application can be found at the end of this handbook.

BEAUTICIAN / BARBER SERVICES

Beautician and barber services are available. Arrangements can be made for services through the Resident Personal Account Office (please refer to the Resident Personal Account section of this handbook). A form is included in this handbook.

LAUNDRY

The Chautauqua County Home will provide each Resident with laundry service unless the family prefers to launder clothes privately. Delicate fabrics and articles requiring dry cleaning are not recommended. Dry cleaning is not included in the Chautauqua County Home rate and is not a service that is coordinated by the facility. The following is a list of the laundry days for each unit. All clothes are laundered, pressed and mended (if needed) and returned to the unit on the assigned day.

MAIL

Personal mail, packages, and flowers, which are sent to the Resident, will be delivered daily. Business mail can be forwarded to the Resident’s Designated Representative or handled by the Billing/Business Office upon request. Residents may purchase postage through the Resident Personal Account Office. A mailbox is located in the lobby. Assistance is available for those requesting it such as opening mail or addressing envelopes.

NEWSPAPER

Newspaper delivery, for an additional fee, can be arranged by contacting the Resident Account Office.

SHOPPING

A Country Store, which stocks small personal items and snacks, is open weekly for shopping. Although the Chautauqua County Home provides for most personal care items, Residents enjoy having the opportunity to shop independently and the freedom to purchase and use “favorite” products.

TELEPHONE SERVICE

Telephones are located in the visitors lounge, lobby, and on each nursing unit for Resident use. Telephones are wheelchair accessible and useable by Residents who are visually and hearing impaired.

TELEVISION

A large screen television is located in the Day Room. In addition, televisions are located on each unit in the TV room. Residents may have a television in their room if they desire. The Maintenance Department inspects all electrical devices for safety.

Cable television service is available for an additional fee. Chautauqua County Home personnel can make arrangements for installation. A cable guide is included in this handbook.
