At Carillon Nursing and Rehabilitation Center, our family has

individuals who are no longer able to live safely within the

Department of Health. Our interdisciplinary team plans and
executes care for each resident based on a holistic approach;
medicine, nursing, dietary, rehabilitation, social services and other
disciplines work together to create the best plan of care for each
resident / patient and their family members.
