The Special Care Center and its team are providing quality long term care to geriatric and disabled residents. As an integral part of the Bronx-Lebanon Hospital Center Health Care System, this 240 bed facility offers a full range of programs, including medical, dental, psychological, and hospice care (a new addition).

It also features physical, speech, and occupational therapies; social service programs; on-site radiology; a pharmacy, and even a beauty salon. The Special Care Center was one of the first in the nation to have dedicated AIDS units within a traditional long-term care setting.

As a result of a recently completed major modernization, nursing stations and resident floors were further upgraded and a state of- the-art computer system (linking the Special Care Center to the hospital) was put into place. Lobby and security desk areas were expanded and a new meditation chapel was also added.

ADMISSIONS

The Admissions Department serves as the point of entry into the Special Care Center. Our facility prides itself on being "resident focused" and practices a philosophy that embraces customer service, comprehensive care, value for the resident's quality of life and dignity.

The immediate service area encompasses people who live in the Bronx, Manhattan, Queens, Brooklyn and Staten Island. However, due to medical need, family considerations and other extenuating circumstances, exceptions are made.

We have admitted residents from Long Island, from parts of New York State and from as far away as California. BLSCC is a residential facility, where all services are delivered on-site and when necessary patients are referred to Bronx-Lebanon Hospital Center.

Amenities Welcome Package: All newly admitted residents are presented with a Care Package, hand delivered by staff from the Admissions Department.

Banking Services: In-house teller window is open Monday through Friday from 9:00am to 12:00 pm. The Teller's window is closed on weekends and legal holidays.
Outgoing/Incoming Mail For our residents' convenience, mail is picked up/dropped off Monday through Friday.

ACTIVITIES

The Activities Department also known as therapeutic recreation prompts amazing results from our residents who are convalescing. We see short and long-term residents benefit through a variety of recreation and creative arts therapies geared to improve a resident's quality of life. These modalities address the full gamut of physical, emotional, social, and spiritual needs of each resident.

We provide a healing environment in which positive relationships are built, self-esteem is raised, and life skills are more fully developed, enabling a person to continue to lead a productive and independent life. Activities is the place where legends are rediscovered, talent is nurtured and a budding dream on a small stage can come true - residents experience opportunities to act, sing, tell jokes, recite poetry, do plays/musicals, play bingo, participate in jeopardy, participate in dance contests, horticulture club, participate in Resident Council and much, much more!

The department has an activities committee comprised of residents that meet regularly with staff to discuss planning of events and presentations. The department has a diverse staff to ensure a resident's "cruise to nowhere" is full of entertainment. The director is a certified music therapist, who is confident and open-minded. On staff there is also a registered dance therapist, dance instructor, visual artist, board certified music therapist, certified cosmetologist and events planner.

DENTAL

The Department of Dentistry provides a unique characteristic of direct comprehensive patient care services 8-hours a day Monday-Friday on the premises. Our ability to provide this care on a continuous basis sets us apart from an external dental clinic.

Professional dental care is provided by highly skilled and recognized general practitioners that posses a minimum of one year of the general practice residency training in a hospital setting. The residents at Special Care are afforded dental procedures such as extractions, removable dentures, cosmetic bonding, cleanings, oral hygiene instructions and on occasions placements of endosteal implants.

DIETARY

The Food and Nutrition Services Department at the Special Care Center is constantly praised for delicious meal preparation and the variety of healthy choices offered. All meals are prepared on the premises. Unlike many other long-term care facilities, our residents receive 3 home-cooked meals per day with snacks in between meals.

Our residents experience significant positive nutritional outcomes as they transition and adjust to renal diets, conquer wasting syndrome, obesity and manage diabetes. Our team consists of our Food Service Director, who is a graduate of the renown Culinary Institute of America, a Clinical Nutrition Manager, two registered dietitians, a group of wonderful dietary supervisors, in addition to cooks and dietary aides who are responsible for the tremendous success of the department.

Once a month, the Food Committee, a group of residents who consult with the Food Services Director on meal planning meet to discuss concerns/changes/ requests they may have. Our residents are a part of the decision-making process concerning their nutritional status.

The diets are liberalized to facilitate maximum intake, and every month the dietitians conduct a survey to determine how well our services meet the residents' needs. In tandem with the Activities department, Dietary provides meals to enhance special cultural observances/holiday meals, birthday lunches, summer outdoor barbecues and more.

HOSPICE

When it comes to making end of life decisions, a resident's self-determination and comfort are of the highest priority at the Special Care Center . The comprehensive care plan team works closely with the resident and their loved ones to ensure the highest quality of life and support services are available to accommodate the resident's needs.

PHARMACY

Our Pharmacy is located on-site at the Special Care Center. The Pharmacist is available to residents for consultation, questions and advice. There are 6 physicians on staff who work very closely with the pharmacy department.

PHYSICAL THERAPY

The Rehabilitation Department at the Special Care Center consists of a team of highly dedicated professionals in Physical, Occupational and Speech therapy.

We are fully equipped to treat a range of injuries, address general Orthopedic issues and Neurological conditions. Treatment of Cerebrovascular Accidents, Hip Replacement, Knee Replacement, Motor Vehicle Accidents, Hand Injuries, swallowing problems, and several types of speech disorders are some of the common conditions we treat daily.

SUBSTANCE ABUSE

The Substance Abuse Treatment Services (SATs) at the Special Care Center is a voluntary program. Our aim is to help residents understand and cope with their diagnosis, social issues and related addictions by providing them with a safe nurturing environment that is structured. In partnership with Bronx-Lebanon Hospital , our treatment services have a vast range of Oasis-supported services including Methadone maintenance, detoxification regimens, smoking cessation and other strategies that minimize or eliminate substance abuse.
