ALZHEIMER'S & DEMENTIA CARE

Caring for someone with Alzheimer’s disease or dementia requires patience, expertise and specialized treatment. That is why in 1985 Broadlawn Manor created Long Island’s first dedicated program to provide the compassionate care your loved one deserves.

The Alzheimer’s and Dementia Care program is comprised of two discrete units for residents based on their cognitive abilities and behavior. Both units feature a specialized interdisciplinary team of medical professionals including physicians who are board certified in geriatric medicine and psychiatrists who are board certified in geriatric psychiatry. Working together, they provide individualized care that enhances the quality of life for each resident by maximizing functioning and minimizing frustration.

Our distinctive Therapeutic Recreation Program is designed to build self-confidence and enrich lives through social interaction, mental stimulation, creativity, expression, satisfaction and fun. Activities for each individual are based on their unique interests, cognition and physical abilities, and are managed by licensed therapists and recreation professionals.

Broadlawn Manor provides a safe and secure environment for residents, and peace of mind for family and caregivers. Each unit in the Alzheimer’s and Dementia Care program features protected entrances and exits to safeguard residents, and safety equipment to keep them from harm. Broadlawn Manor is also the first facility on Long Island to implement CareTracker, a computerized documentation system that enhances resident treatment through enhanced accuracy and accountability.

Broadlawn Manor partners with the Alzheimer’s Association Long Island to provide programs such as Safe Returns and Memories In The Making, as well as supporting legislative advocacy.

SKILLED NURSING

For more than 50 years, the Skilled Nursing Program at Broadlawn Manor has been dedicated to providing long-term care for residents with dignity, compassion and respect. The 320-bed facility offers comfortable private and semi-private accommodations, a pleasant dining experience, friendly common areas and beautifully landscaped grounds.

The individualized care for each resident is overseen by an experienced interdisciplinary team of medical professionals that encompasses physicians board-certified in geriatric medicine, registered nurses, certified nursing assistants and social workers. Working together, they provide progressive and innovative 24-hour care, with the goal of enhancing the quality of life for each resident.

The specially trained staff assists residents with activities of daily living to improve their quality of life. And Broadlawn Manor is the first facility on Long Island to implement CareTracker, a computerized documentation system that enhances resident treatment through enhanced accuracy and accountability.

INPATIENT SUB-ACUTE AND REHABILITATION CARE

Broadlawn Manor’s Sub-Acute and Rehabilitation program offers a full range of inpatient medical care, nursing services and advanced rehabilitative programs for adults 18 years of age or older. The modern rehabilitation facility at Broadlawn Manor features an experienced team of medical professionals along with state-of-the-art equipment and innovative approaches. The goal of the Sub-Acute and Rehabilitation Care program is to return individuals to their highest level of function and maximize their potential to attain independence in order to improve their quality of life and return to the community.

Our approach centers on the collaboration of a multidisciplinary team of experienced, board certified physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians and physical occupational, recreational and speech therapists. Each patient’s medical, social and emotional needs are met through an individualized care plan, which is monitored by each member of the team and adjusted as necessary to promote recovery.

Broadlawn Manor’s distinctive Therapeutic Recreation Program offers activities designed to build self-confidence and enrich lives through social interaction, mental stimulation, creativity, expression, satisfaction and fun. The activities for each individual are based on their unique interests, cognition and physical abilities, and are managed by licensed therapists and recreation professionals.

Discharge planning takes place throughout treatment, and focuses on the patient’s return to the community, and the coordination of any necessary outpatient services, community resources, government entitlements and durable medical equipment.

Sub-Acute Care

Broadlawn Manor’s Sub-Acute program provides medical treatment to patients transitioning from hospital to home. Our healthcare professionals have tremendous expertise providing a broad spectrum of medical and post-surgical care such as pneumonia, wound care management, coronary by-pass surgery, cardio-thoracic surgery, pain management, vascular surgery, tracheotomies, as well as intravenous and respiratory therapies.

Rehabilitation

Broadlawn Manor’s Rehabilitation program provides intensive physical, occupational and speech therapy to patients recovering from strokes, fractures, joint replacements, amputations or neurological conditions. Our licensed therapists are focused on promoting independence through a variety of rehabilitation therapies and treatments. They provide education, motivation and support needed for patients to attain a peak-level of functioning upon discharge. Broadlawn Manor also offers Outpatient Rehabilitation for those who are able to live within the community.


OUTPATIENT REHABILITATION

Broadlawn Manor’s Outpatient Rehabilitation program offers a full range of advanced rehabilitative programs for adults 18 years of age or older. Treatments include Physical Therapy, Occupational Therapy and Speech Therapy for patients recovering from joint replacements, general deconditioning, cardiac events or balance disturbances.

The modern rehabilitation facility at Broadlawn Manor features an experienced multidisciplinary team of licensed medical professionals, state-of-the art equipment and innovative approaches. Working together, they are focused on promoting recovery through a variety of rehabilitation therapies and treatments. Each patient receives the individualized care, motivation and support needed to attain a peak-level of functioning.

Broadlawn Manor is proud to be the first Skilled Nursing Facility in Suffolk County to offer VitalStim Therapy.â„¢ VitalStim Therapy is a non-invasive external electrical stimulation therapy that reeducates the throat muscles needed for swallowing. This innovative therapy has assisted numerous patients in improving or relieving swallowing difficulties and is administered by Speech-language therapists certified in the technique. Transportation may be available depending on the type of insurance and coverage.


ADULT DAY HEALTH SERVICES

Broadlawn Manor’s Adult Day Health Services program provides a breadth of health and social services to physically, socially and/or mentally impaired individuals who are 30 years of age or older. The program is designed for individuals who will benefit from preventative, diagnostic, therapeutic, rehabilitative or palliative services, but who do not require continual inpatient care. The program’s goal is to improve each individual’s wellbeing and independence in activities of daily living, while offering respite for caretakers.

Upon enrollment, each individual receives a comprehensive screening by a physician, nurse and social worker. Included in the screening is an evaluation for the need of ancillary healthcare services, which are provided through the program by experienced dentists, podiatrists, audiologists, optometrists, speech pathologists, psychiatrists and psychologists.

Broadlawn Manor’s approach to health and social services centers on the collaboration of a multidisciplinary team of healthcare professionals — each with expertise and special training. The team includes a medical director, nursing staff, social workers, occupational and physical therapists, recreational specialists, dieticians and pharmacists.

Participation in the program’s many daily activities is encouraged. The activities are designed to enrich lives through social interaction, mental stimulation, creativity, expression, satisfaction and fun. Examples of some of the activities available include:

- Cooking

Nutritious choices are offered for continental breakfast, hot lunch and daily snacks. If necessary, each individual’s medication can be monitored and dispensed as prescribed.

The Adult Day Health Services program operates seven days per week from 9:00am to 3:00pm. Door-through-door transportation is available within the catchment area for those unable to make their own arrangements.


THERAPEUTIC RECREATION

Therapeutic Recreation plays an integral role throughout all of the programs at Broadlawn Manor, and provides an exciting full day of activities that improve quality of life, encourage self-expression, cultivate wellness, promote self-esteem and increase social interaction.

All activities are thoughtfully designed with our patients and residents in mind. The goal of Broadlawn Manor is to enhance quality of life through activities that will meet an individual’s age, cognitive and physical abilities and interest.

For individuals diagnosed with Alzheimer’s or related dementias, unique programs are designed to promote communication, expression, reminiscence and socialization. Broadlawn Manor has also partnered with The Alzheimer’s Association in offering “Memories in the Making”, a program that utilizes art therapy in an innovative way to enhance creativity and self-esteem.

Broadlawn Manor recognizes how important families and friends are to patients and residents. Visiting is encouraged and the Annual Family Day Celebration is a fabulous event held each September.

Dream Team Program

A unique program offered annually — made possible through fund raising — to afford our patients, residents and families an opportunity to support a special dream. Dreams that indulge the imagination and empower the spirit. Dreams may include a trip to Yankee Stadium, a fishing trip on the Great South Bay, trying the slot machines at Mohegan Sun or attending an English Tea Party. The dreams of our patients and residents become our dreams.

Golden Games

Broadlawn Manor participates each year in the Golden Games, which is an all-day athletic competition for senior citizens in Suffolk County. Tournaments include the wheel chair relay, bean bag distance throw, volleyball, and bowling.
