Welcome to Bridgewater Center for Rehabilitation and Nursing. For almost four decades, we have served our community with compassionate and skilled nursing care.

Our team of caring professionals deliver a wide range of services designed to return our residents back to the community or to restore them to as great a degree of independence as possible.

Rehabilitation - Those seeking rehabilitation to recover from a serious injury or illness will find skilled therapy teams that provide Speech, Occupational and Physical Therapy Rehabilitation services.

Dementia residents are cared for with tender compassion in a safe and secure environment.

Designed to support both our residents and their families, our Respite, Palliative and Hospice, care is provided by trained professionals. And for those residents needing the most intensive medical care and supervision, Bridgewater's skilled around-the-clock nursing department is among the area’s best.

The new management team is committed to improving on the quality care and service that the 356 bed facility offers, as well as to improving Bridgewater’s physical structure through an extensive remodeling and refurbishing project. Our vision remains unchanged: to reaffirm the tradition of a community that cares and supports the emotional and spiritual growth of our residents, as well as their physical well being.

RESPITE, PALLIATIVE & HOSPICE

Our Respite Program provides for the short-term care of your loved one.

- Designed to give caregivers temporary relief from the demands and responsibilities of taking care of a loved one,
- Bridgewater’s staff of trained professionals provide all of the personal care and nursing assistance needed in a secure, friendly atmosphere.

The Hospice Program at Bridgewater is designed to

- Support both the resident and the family with skilled and compassionate care by a staff that is thoroughly trained to respond to the special needs of our hospice residents.
- We provide medical management, skilled nursing care, comfort care and therapies.
Medication management

We always encourage family support and involvement in the resident’s care. Our goal is to provide the services needed to improve the quality of life for terminally ill residents so that they can live the remainder of their lives as comfortably and peacefully as possible.

Palliative Care at Bridgewater provides

- Supportive care to residents who have chosen to forego further active medical care for various reasons.
- We treat the symptoms of a life-threatening illness, keep the resident as comfortable and pain free as possible and assist the family whenever needed.


REHABILITATION

Serious injuries require state of the art restorative therapies and training equipment. As the area’s largest Rehabilitation Center, Bridgewater Center for Rehabilitation and Nursing is noted for it’s

- premier delivery of service, restoring our residents back to health and returning them to the community and their families.

Our goal is to help residents to regain functional independence after a loss caused by a medical or surgical event. Designed to achieve the highest level of independence, we provide our residents with

- a combination of physical, occupational and speech therapy, psychiatric counseling and social work, all directed towards helping an individual to maintain or recover their physical capabilities.

When a complex problem requires rehabilitation expertise over several disciplines ,our Rehabilitation team can develop

- a multi-phased program to coordinate therapy.

We believe that age alone is not a reason to postpone or avoid rehabilitation and have had great success with impaired elderly residents who have participated in our rehabilitation programs.

DEMENTIA CARE

When Alzheimer’s dementia strikes a loved one, it can be over-whelming to a caregiver. At Bridgewater Center for Rehabilitation and Nursing, we have designed a program to meet the special needs of our residents with Alzheimer’s disease or dementia.

- We offer a safe environment, balanced with state-of-the-art emergency and wander control systems for the protection of our residents and your peace of mind.
- Our caring and experienced staff is available 24 hours a day to provide for the needs of our Alzheimer and dementia residents, regardless of their unscheduled and varying health care needs.
- We provide assistance when it’s needed for tasks that have become too complicated, calm assurances when confusion and fear disorient, and an accepting environment where it’s perfectly fine to wander, enjoy a friendly chat or share a smile.
- We also know how important it is to support the family caregivers as a loved one’s personalities change and connections become more fragile.


Comprehensive Care Planning

Upon admission, the treatment team evaluates the resident’s condition and needs, and devises a Comprehensive Care Plan detailing each discipline’s interventions to meet the resident’s holistic needs. The goal is to promote the resident’s health and function at the maximum attainable levels. The team meets quarterly and upon any significant change in the residents’ status. The resident and /or their family are invited to attend these meetings to take part in the comprehensive care plan.


DINING & ACTIVITIES

Dining

It is important for our residents to dine in comfort and with dignity. With this in mind, we provide dining supervision by our staff, but are respectful of the specific needs of the individual.

- Our dieticians prepare three nutritious, balanced meals and snacks each day with consideration to special diets and preferences.
- Our residents are encouraged to participate in their own meal planning.
- Each unit has a pantry for “anytime” snacks.
- Holiday and special event menus are provided throughout the year.
