Welcome to the Albany County Nursing Homeand Department of Residential Health Care Facilities.

Our Mission:

The Department of Residential Health Care Facilities provides excellence in long term care services to the people of Albany County and surrounding areas. The Department will provide a nurturing atmosphere staffed by qualified and compassionate individuals who are able to offer innovative health care through complex medical services and a comprehensive approach. The Department's vision is a community integrated health care continuum in which every individual is a valued member.

Facilities:

The Commission on Health Care Facilities, better known as the Berger Commission, was established by the Governor and Legislature of the State of New York to conduct an independent review of the health care system in New York State. Its primary task was to ensure that the regional and local supply of hospitals and nursing homes throughout the State was adequate to meet community needs for high quality, affordability, and accessibility.

As a result of the Berger Commission's findings, Albany County was targeted as an area needing facility downsizing. As a direct result of the Berger Commission, the Ann Lee Home and the Albany County Nursing Home were to be consolidated into one operating entity. The consolidation, which led to the closure of Ann Lee Home in January 2008, also called for downsizing Albany County Nursing Home to a capacity of 250 beds.

Albany County Nursing Home continues to successfully operate as a skilled nursing facility providing comprehensive clinical programming to those it is privileged to serve. Albany County Nursing Home offers a complete panoply of services including 24 hour skilled nursing care; physical, occupational, respiratory, and speech therapies; nutritional programs developed and overseen by registered dietitians; social services; spiritual programs; recreational activities; clinic services; psychological services; staff physicians; spiritual care; volunteer programs; as well as the daily amenities one would expect including housekeeping and laundry service.


CARE AND SERVICES

Albany County Nursing Home is a licensed and certified provider of skilled nursing services offering care for those who may pay privately, those covered by Medicare, those covered by Medicaid, those with Veterans' Benefits, and for many with third party insurance or HMO coverage.

Albany County Nursing Home is staffed by qualified professionals who diligently work in an interdisciplinary environment to provide 24 hour skilled nursing care; physical, occupational, respiratory, and speech therapies; nutritional programs developed and overseen by registered dietitians; social services; spiritual programs; recreational activities; clinic services; psychological services; staff physicians; spiritual care; volunteer programs; as well as the daily amenities one would expect including housekeeping and laundry service.

Additionally, the Albany County Nursing Home strives to assist its residents to achieve and maintain their highest, individual levels of both physical and psychosocial functioning. Individualized plans of care are developed for each resident and are regularly reviewed as part of the Facility's Clinical Review process. Moreover, the Facility encourages the participation of both the resident and family in the process of developing individualized plans of care.

Albany County Nursing Home also offers a number of creative programs designed to not only offer diversity, but also to progressively address the needs of some of its special populations. This may be best exemplified by the Facility's new RISE Program which is being developed for implementation in early 2008. The RISE Program, which represents Renaissance Integrating Structured Enhancement, will be designed to be a special in-house day program for those diagnosed with mild and/or moderate stages of Dementia of the Alzheimer's type.

Facility programs emphasize the sensitivity of a caring environment and quality of life. Among our many goals is to offer a pleasing environment to our residents, one that encourages and facilitates satisfaction, rehabilitation, and social interaction. While the delivery of service within a medical model remains of paramount importance, the Facility also strives to offer quality services that enhance the resident's social well-being. Recreational activities, non-denominational spiritual programs led by a staff pastor, and our active volunteer program all help in achieving that balance.


Clinical Programming and Services

Our Medical Staff
Patients and residents are free to choose their own physician (who must apply for staff privileges and undergo a credentials review), or have one of our staff physicians oversee their care and treatment.

Department of Nursing
Albany County Nursing Home provides skilled nursing services twenty-four hours per day, seven days per week. Our facility boasts one of the highest nursing to patient staffing ratios found in the industry today.

Department of Rehabilitation Services
Albany County Nursing Home offers comprehensive therapeutic programming through our Department of Rehabilitation Services.

Activities and Therapeutic Recreation
Albany County Nursing Home is fully committed to offering our residents and patients a social environment that provides a milieu of both diversional and therapeutic activity.

Social and Psychosocial Services
Each patient or resident is assigned a social worker upon admission. Our Department of Social Services assists each resident to ensure his or her general daily needs are met.

Pastoral Care
Albany County Nursing Home recognizes the importance of spirituality in daily life. We retain the services of a full-time pastor who is always available for personal counseling and the spiritual needs of our residents.

Food and Nutrition
All facility meals and menus are planned by a registered dietitian to ensure they meet the nutritional and dietary needs of each patient or resident.

The RISE Program
Our new and unique RISE ("Renaissance Integrating Structured Enhancement") program is a clinically-based program designed for residents diagnosed with mild or moderate dementia.

Ancillary Services
Albany County Nursing Home provides a number of support services to ensure our patients and residents are able to maintain a high quality of life.
